Here’s what to do:
1) Download the Mail-In-Rebate form from the link above.
2) Buy one Scope Outlast 750 ml or Scope Mint 1 L
3) Mail the original rebate form, original UPC from the package (bar code), Original dated sales receipt with the store name and product purchase price circled in a stamped envelope to the address on the form (located on the left side).
4) Wait 6-8 weeks for the delivery of your prepaid card via mail.
